Join a compassionate healthcare team as an On-Call Registered Nurse providing skilled nursing care during after-hours shifts. This role involves responding to clinical and non-clinical situations five call shifts per week, delivering expert nursing assessments, interventions, and supportive care tailored to hospice and home health patients and their families. You will collaborate closely with physicians, RN case managers, and other healthcare professionals to optimize patient outcomes.

Essential Functions:
- Triage calls and conduct after-hours visits for hospice and home health patients across the regional Midwest area.
- Perform thorough evaluations of patient needs and develop comprehensive care plans.
- Adhere to current hospice and home health nursing standards and agency protocols.
- Conduct timely home or facility visits to ensure care aligns with individualized plans for patients ranging from newborns to geriatrics.
- Maintain clear communication with physicians, patients, families, and care teams regarding patient status and care adjustments.
- Educate patients and families on pain management, symptom control, disease processes, and end-of-life signs.
- Administer and manage medications accurately and efficiently.
- Complete hospice and home health admission assessments and supervise home health aide services.
- Participate in scheduled call duties to ensure 24-hour nursing availability.
- Document all care activities promptly and accurately.
- Foster positive relationships with patients, families, colleagues, and referral sources.

- Graduate of an accredited nursing program.

Preferred Qualifications:
- Bachelor of Science in Nursing (BSN).
- Previous hospice or home health experience.
- Experience with Home Health OASIS Data Set.

Licensure and Certifications:
- Current Registered Nurse license in the state of Kansas.
- Basic Life Support (BLS) certification within three months of hire.
- Valid Kansas driver’s license and auto insurance meeting state requirements.

Preferred Certifications:
- Hospice and Palliative Care Nurse Certification.
- Certified Wound Care Nurse.
- Certified OASIS Specialist.
