Join a compassionate care team as a Social Worker, providing vital advocacy and psychosocial support to patients and families receiving hospice, home health, and private duty services in the Central Plains region of the United States. You will play a key role in promoting wellness and comfort through comprehensive care planning, community resource referrals, and end-of-life support.

Education: Completion of an accredited Bachelor of Social Work (BSW) program is required.

Experience: Preferred candidates will have 1-3 years of experience in medical facilities, hospice, or home care settings.

Requirements: Current licensure as a BSW in the relevant state, Basic Life Support (BLS) certification within three months of hire, and a valid driver's license with appropriate auto insurance.

As part of your role, you will conduct thorough psychosocial assessments during home visits, develop individualized intervention plans, and collaborate closely with interdisciplinary teams to address social, emotional, cultural, and spiritual needs. You will advocate for patients' rights and decisions, facilitate advance care planning including healthcare proxies and living wills, and maintain respectful, dignified relationships with patients and families.
