Join a dedicated Assertive Community Treatment (ACT) Team that supports individuals with severe and persistent mental illness who have not found success with traditional clinic-based services. This role involves outreach, enrollment, and providing comprehensive mental health and social service support to help clients secure and maintain housing and work towards a fulfilling life within their community. Collaborate closely with clinicians, medical providers, and support staff to deliver intensive, personalized care aimed at keeping consumers engaged and supported in their regional community.

Essential Functions: Utilize evidence-based, trauma-focused, and person-centered case management techniques to address symptoms of mental illness and enhance emotional and functional well-being. Develop crisis prevention plans and treatment goals in partnership with consumers. Coordinate care with community partners, caregivers, physicians, and other providers. Support clients in achieving treatment goals, medication education, crisis management, and daily living activities. Assist with appointment attendance and discharge planning to ensure smooth transitions. Maintain timely and accurate documentation and participate in continuous quality improvement efforts. The schedule includes varied hours, including some evenings, weekends, and on-call coverage to meet consumer needs during crises.

Education: Bachelor's degree or equivalent experience in a related field is required. Preferred degrees include Social Work, Sociology, Psychology, or Special Education with specialized training in human development and family dynamics.

Experience: Three to five years of professional experience in mental health services, crisis response, or related fields is preferred. Experience educating clients on self-care and improving daily living activities is highly valued.

Requirements: Completion of CPST Case Management online training, KHS online training, background checks including KBI, child and adult abuse registries, and motor vehicle screening. Adult Case Management training must be completed within 60 days of employment. A valid driver's license, insurance, and reliable vehicle are necessary.
