As a dedicated healthcare professional, the Nurse Navigator guides patients through the complexities of the healthcare system, from diagnosis to treatment and sometimes end-of-life care. Acting as a vital link between patients and clinical teams, this role provides personalized support to patients, families, and caregivers. Responsibilities include connecting patients with medical and social resources, coordinating follow-ups for appointments and referrals, and communicating patient progress to healthcare providers and agencies.

Essential Functions

• Support patients throughout their healthcare journey
• Educate patients on diagnoses, treatments, and potential side effects
• Assist patients in overcoming barriers such as financial challenges or transportation issues
• Provide reliable resources and information
• Manage timely scheduling of tests and procedures

Education and Experience

• Graduate of an accredited nursing program
