Take charge of developing, implementing, and managing personalized care plans for hospice patients ranging from newborns to the elderly. Collaborate closely with physicians and interdisciplinary teams to optimize patient outcomes and ensure compassionate, patient-centered care.

Key Responsibilities:

• Create individualized care plans based on medical and nursing diagnoses and patient goals.
• Deliver skilled nursing services aligned with the established care plan.
• Focus on symptom management and comfort to support patient and family care objectives.
• Communicate assessments and care updates regularly with hospice staff and volunteers.
• Coordinate with physicians, patients, families, and other disciplines to adjust care plans as needed.
• Supervise Home Health Aide services in accordance with policy.
• Maintain timely and accurate documentation following agency guidelines.
• Foster positive relationships with patients, families, colleagues, physicians, and referral sources.
• Observe and address patients’ physical, mental, emotional, and spiritual needs collaboratively.
• Adhere strictly to HIPAA regulations to protect patient information.

Education and Experience:

• Graduate of an accredited nursing program.
• Several years of clinical nursing experience preferred.
• BSN degree is advantageous.
• Previous hospice or home health experience is highly valued.
• Certification as a Hospice and Palliative Care Nurse is preferred.

Licenses and Certifications:

• Current Registered Nurse license in the relevant state.
• BLS certification required within three months of hire.
• Valid driver’s license and proof of auto insurance as required.
