Take charge of developing, implementing, and managing individualized patient care plans in a hospice setting. Collaborate closely with physicians and interdisciplinary teams to optimize patient outcomes and provide compassionate end-of-life care. Engage actively in delivering exceptional customer service while fostering respectful relationships with patients, families, and colleagues.

Essential Functions

• Create personalized care plans based on medical and nursing diagnoses for patients ranging from newborns to geriatrics in hospice care.
• Deliver skilled nursing services aligned with the established care plan, focusing on symptom management and patient comfort.
• Maintain clear communication of patient assessments and care plans with hospice staff and volunteers regularly.
• Work collaboratively with physicians, patients, families, and other disciplines to adjust care plans as needed.
• Supervise Home Health Aide services according to policy guidelines.
• Ensure timely and accurate documentation in compliance with agency standards.
• Build and sustain positive relationships with patients, families, healthcare providers, and referral sources.
• Observe and assess patients’ physical, emotional, mental, and spiritual conditions, coordinating with team members appropriately.
• Adhere strictly to HIPAA regulations protecting patient health information.

Minimum Knowledge and Skills

• Proficiency with MS Office and keyboarding skills.

Education and Experience

• Graduate of an accredited nursing program.
• Several years of clinical nursing experience.
• Preferred: Bachelor of Science in Nursing (BSN).
• Preferred: Previous hospice and/or home health experience.
• Preferred: Certification as a Hospice and Palliative Care Nurse.

Licenses and Certifications

• Current Registered Nurse license in the relevant state.
• BLS certification required within three months of hire.
• Valid driver’s license and proof of auto liability insurance as required.
