Join a compassionate care team as a Social Worker, providing vital patient and family advocacy along with psychosocial support for individuals receiving hospice, home health, and private duty services. This role focuses on enhancing patient and family wellness through improved care outcomes, advance care planning, community resource referrals, and end-of-life care guidance. You will actively engage in delivering exceptional customer service and fostering respectful relationships with patients, families, and colleagues.

Essential Functions: Conduct thorough psychosocial assessments during home visits, addressing social, cultural, emotional, and spiritual needs. Develop and implement focused intervention plans in collaboration with interdisciplinary teams, ensuring holistic care that respects patient preferences. Continuously collect and analyze data to improve safety, satisfaction, and care effectiveness. Serve as a liaison between outreach programs, hospice, home health, long-term care facilities, and community services. Facilitate advance care planning including healthcare proxies, living wills, and power of attorney documentation. Maintain up-to-date knowledge of clinical trends, regulations, and payer requirements. Document all activities accurately and comply with privacy regulations regarding protected health information.

Education: Completion of an accredited Bachelor of Social Work (BSW) program is required. A Master of Social Work (MSW) is preferred.

Experience: Preferred 1-3 years of experience in medical facilities, hospice, or home care settings.

Requirements: Current licensure as a BSW in the relevant state, Basic Life Support (BLS) certification within three months of hire, valid driver’s license, and auto insurance meeting state requirements.
