Join a dedicated Assertive Community Treatment (ACT) Team serving individuals with severe and persistent mental illness who have not found success with traditional clinic-based services. This role involves outreach, enrollment, and providing comprehensive mental health and social support to help clients maintain housing and reintegrate into fulfilling community lives.

Key Responsibilities:

• Conduct comprehensive assessments for new clients entering the ACT program and determine appropriate services.
• Deliver evidence-based, trauma-informed, and person-centered treatments including wellness self-management counseling and integrated dual disorder care.
• Collaborate with the ACT Team to develop crisis prevention plans and individualized treatment goals.
• Coordinate care with community partners, families, physicians, and other providers to support client progress.
• Support clients in achieving treatment goals, medication education, crisis management, and daily living skills.
• Participate in discharge planning and facilitate smooth transitions to community or higher levels of care.
• Provide culturally competent services primarily in community settings where clients feel most comfortable.
• Maintain timely and accurate documentation and complete required trainings.
• Participate in varied scheduling including some evenings, weekends, and on-call rotations to support clients during crises.

Education: Master’s Degree in Social Work, Psychology, or Marriage and Family Therapy is required.

Experience: Preferred 3-5 years working with severe and persistent mental illness (SPMI/SMI) or serious emotional disturbance (SED) populations, including diverse and high-risk groups. Experience integrating care with other providers is highly valued.

Licensure & Certifications: Must hold a BSRB Masters Level Clinician license and be qualified as a Mental Health Professional (QMHP). Valid driver’s license required. Within 6 months of hire, obtain temporary Masters Addiction Counselor licensure and full licensure within 12 months. Clinical Addictions Counselor licensure is preferred.
