Description

• Conduct thorough patient evaluations across inpatient or outpatient settings and develop individualized, goal-oriented treatment plans
• Deliver physical therapy interventions targeting mobility, functional strength, coordination, and balance restoration
• Work alongside physicians, nursing staff, case managers, and fellow rehabilitation specialists to drive optimal patient outcomes
• Provide meaningful education to patients and their families regarding recovery goals, home exercise programs, and safe discharge transitions
• Maintain accurate, timely clinical documentation in alignment with facility standards and regulatory requirements
• Engage actively in interdisciplinary care conferences and team rounds to ensure seamless, coordinated care delivery

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• Active state licensure as a Physical Therapist in good standing
• CPR certification preferred
• Specialty certifications such as neurological or geriatric rehab are welcomed, though not required

Skills

• Prior clinical experience strongly preferred
• Demonstrated ability to manage patients presenting with complex medical conditions and mobility challenges
