This role focuses on providing a supportive, normalizing experience that enhances overall functioning while helping individuals remain in the least restrictive environment possible—their home and community.

Essential Responsibilities

• Provide skill training on targeted goals with measurable outcomes as determined by a Qualified Mental Health Professional.
• Deliver intensive, community-based mental health services using a strengths-based, skill-building approach.
• Collaborate with clients, families, and treatment teams to develop and maintain client-centered treatment plans.
• Help clients understand their symptoms and explore possible solutions.
• Assist clients and families in engaging natural community supports to develop effective coping strategies.
• Coordinate mental and physical healthcare services, facilitating communication among clients, providers, families, and community partners.
• Adhere to privacy and security regulations regarding Protected Health Information (PHI).
• Maintain professional and effective communication with clients, team members, and external agencies.

Education and Experience

• Required: Bachelor’s degree in Social Work, Psychology, or related field; or Associate's degree plus 2+ years of experience educating clients on self-care and daily living activities; total education and experience must equal 4+ years.
• Preferred: Bachelor’s degree in Social Work, Sociology, Psychology, or Special Education with specialized training in human development and family dynamics.
• Preferred experience: 3-5 years in mental health services, crisis response, or related fields.

Licenses and Certifications

• Required: CPST Case Management training, KHS training, background checks including KBI, child and adult abuse registries, motor vehicle screening.
• Completion of Adult Case Management training within 60 days of hire.
• Valid driver’s license, insurance, and reliable vehicle if driving is required.

Skills and Abilities

• Familiarity with community mental health services.
• Strong organizational skills.
• Intermediate proficiency with Microsoft Office.

Physical Requirements

Light work involving occasional exertion up to 25 lbs and frequent exertion up to 10 lbs. Standing or walking may be required 11-25% of the day.
