Essential Functions

• Coordinate care including needs assessments, identifying care options, and communicating with patients and families within an interdisciplinary environment.
• Collect and manage patient data related to case management, care planning, and quality improvement analysis.
• Monitor care delivery through patient rounds, documentation, and progress tracking toward desired outcomes.
• Maintain ongoing communication with utilization review staff and perform utilization reviews for all patients.
• Support nursing staff with patient care as needed and assist in rehabilitation training and education for staff.
• Comply with healthcare regulations and privacy standards, including HIPAA, ensuring protection of patient information.
• Perform other duties as assigned by leadership.
