Description

• Conduct thorough patient evaluations across inpatient and outpatient settings, developing individualized and goal-oriented treatment plans
• Deliver targeted physical therapy interventions addressing mobility restoration, functional strength, balance, and pain management
• Partner with physicians, nurses, case managers, and fellow rehabilitation specialists to deliver coordinated, high-quality outcomes
• Provide meaningful education to patients and their support systems regarding therapeutic goals, home exercise programs, and discharge readiness
• Maintain accurate, timely documentation of evaluations, progress notes, and treatment records in compliance with facility and regulatory standards
• Actively participate in interdisciplinary care rounds and team conferences to ensure seamless continuity of care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent graduate degree from a regionally accredited institution

Certifications

• Active state licensure as a Physical Therapist in good standing
• CPR certification preferred
