We are seeking a compassionate Licensed Practical Nurse (LPN) to provide dedicated patient care during night shifts in a medical-surgical setting. This role involves delivering care based on the nursing process within the scope of practice, supporting patients' comfort and recovery, and collaborating closely with Registered Nurses and interdisciplinary teams.

Responsibilities

• Provide direct patient care aligned with individualized care plans and under RN supervision.
• Assess patients' physical, psychosocial, educational, and safety needs appropriate to their age group.
• Gather and contribute accurate patient information to support assessments, care plans, and discharge planning.
• Engage patients and caregivers to obtain thorough histories and ensure comprehensive care.
• Deliver compassionate care while maintaining organized nursing processes including assessment, planning, intervention, and evaluation.
• Communicate professionally with patients, families, and healthcare team members to enhance patient experience.
• Adapt positively to rapid changes in departmental needs throughout the shift.
• Report changes in patient conditions promptly to physicians and team members, including diagnostic results and symptom updates.
• Perform therapeutic procedures as assigned and document all nursing interventions and patient responses accurately and timely.
• Provide tailored health education to patients and caregivers related to current diagnoses.
• Assist with patient admissions, discharges, and transfers.
• Identify and address psychosocial needs, coordinating with social services and discharge planners for in-hospital and post-discharge support.
• Recognize and manage potential or actual health risks by initiating appropriate actions in collaboration with RNs.
• Collaborate with interdisciplinary teams to improve patient care quality.
• Seek supervision or assistance when necessary to ensure safe and effective care delivery.
• Maintain continuity of care including medication management and patient instruction.
• Utilize effective interpersonal communication strategies to achieve positive outcomes and satisfaction.
• Leverage internal and external resources to resolve or prevent problems beyond independent management.

Education

Completion of an accredited Licensed Practical Nurse program and current licensure to practice as an LPN.

Experience

Previous experience in a medical-surgical nursing environment is preferred, especially in night shift roles.

Requirements

Strong communication skills, ability to adapt to changing clinical situations, and commitment to compassionate patient care. Must be able to collaborate effectively within a multidisciplinary team.

This position is located in a regional healthcare facility serving the Southern United States.
