Description

• Conduct thorough evaluations for patients in both inpatient and outpatient environments and create tailored treatment plans
• Deliver physical therapy practices emphasizing mobility, strength, stability, and functionality
• Engage with a multidisciplinary team of nurses, physicians, case managers, and other rehabilitation experts to maximize patient outcomes
• Inform patients and their families about therapeutic objectives, discharge processes, and home-based exercises
• Record evaluations, therapies, and progress in line with facility standards and regulatory demands
• Take part in interdisciplinary rounds and care conferences to uphold coordinated patient care

Education

• Doctorate in Physical Therapy (DPT) or equivalent from a recognized institution

Certifications

• Active state licensing as a Doctor of Physical Therapy
• CPR certification preferred
