Description

• Conduct thorough patient evaluations across inpatient and outpatient environments and develop individualized, goal-driven treatment plans
• Deliver physical therapy interventions targeting functional mobility, muscular strength, postural balance, and overall independence
• Work in close collaboration with interdisciplinary team members including physicians, nursing staff, case managers, and fellow rehabilitation professionals
• Provide meaningful education to patients and their support systems regarding therapeutic goals, home exercise programs, and safe discharge transitions
• Maintain accurate and timely documentation of evaluations, sessions, and patient progress in compliance with facility and regulatory standards
• Engage actively in care team rounds and interdisciplinary planning conferences to ensure seamless, coordinated patient care

Education

• Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution

Certifications

• Active state licensure as a Physical Therapist in Michigan
• CPR certification preferred
