Description

• Conduct thorough patient evaluations across inpatient or outpatient settings and develop individualized, goal-oriented treatment plans
• Deliver evidence-based physical therapy interventions targeting mobility, functional strength, balance, and independence
• Partner closely with physicians, nursing staff, case managers, and fellow rehabilitation professionals to drive optimal patient outcomes
• Provide meaningful education to patients and their families around home exercise programs, therapeutic goals, and safe discharge planning
• Maintain accurate, timely clinical documentation in alignment with facility standards and regulatory requirements
• Actively engage in interdisciplinary care conferences and team rounds to support coordinated, whole-person care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• Active state licensure as a Physical Therapist in Michigan
• CPR certification preferred
• Additional specialty certifications in areas such as geriatrics, neurology, or orthopedics are welcomed but not required

Skills

• Prior clinical experience is strongly preferred
• Demonstrated ability to manage patients with complex medical histories and multifaceted mobility challenges
• Excellent interpersonal, communication, and collaborative teamwork skills
