Join a dynamic outpatient primary care team as an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) delivering comprehensive care across the lifespan. This full-time role focuses on managing acute and sick visits for both pediatric and adult patients, while also providing preventive and follow-up care. You will work collaboratively with a group of physicians, providing overflow coverage without managing an independent patient panel. Enjoy a supportive clinical environment with on-site physician availability and no inpatient or after-hours call duties.

Key Responsibilities:

• Evaluate and manage acute and sick visits for pediatric and adult patients
• Perform well child checks and sports physicals
• Conduct Medicare Annual Wellness Exams
• Complete hospital follow-up visits for adults with complex medical conditions
• Order and interpret diagnostic tests, initiate treatment plans, and provide patient education
• Follow up on diagnostic test results, including those received after hours
• Collaborate closely with physicians and support staff to deliver high-quality, team-based care
• Document care accurately and efficiently in the electronic medical record
• Consult with on-site physicians as needed; physician support is always available

Practice Environment:

• Outpatient-only practice with no inpatient responsibilities
• No after-hours call
• Team-based model supporting multiple Internal Medicine and Pediatric physicians
• Collaborative team including experienced Advanced Practice Providers

Education and Experience:

• Graduate of an accredited Physician Assistant or Nurse Practitioner program
• Board certified and eligible for state licensure
• Minimum of 2 years of experience preferred, with strong background in both adult and pediatric care (Family Medicine)
• Proven ability to manage complex patients and a high-volume clinical environment
• Excellent communication, teamwork, and clinical decision-making skills
• Commitment to maintaining required continuing medical education

Licensure and Certifications:

• Certification from the National Commission of Physician Assistants or equivalent for Nurse Practitioners
• Licensed to practice in the relevant state
