Job Summary: We are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) to join an Immediate Care Center serving a regional community in the Midwest. Candidates with prior urgent care, emergency room, or family practice experience with a strong clinical background are preferred. This role involves providing comprehensive assessment, evaluation, planning, and management of patient care for acute and chronic illnesses under physician supervision.

Schedule & Hours: Full-time position with 72 hours bi-weekly, including 1.5 weekends every 4 weeks and 3 holidays per year on a rotating holiday block. The center operates Monday through Friday from 8:00 AM to 7:30 PM, and weekends from 8:30 AM to 4:00 PM, with shortened hours on certain holidays.

Essential Responsibilities:

• Conduct thorough patient evaluations, interpreting lab results, radiographic images, and medical records to arrive at accurate diagnoses.
• Initiate appropriate orders and document patient history, physical exams, assessments, and care plans in medical records.
• Collaborate closely with physicians, medical assistants, and other healthcare professionals to coordinate patient care.
• Provide wellness coaching and education on chronic conditions such as hypertension, diabetes, and depression.
• Refer patients to specialists including diabetes educators, dietitians, and pharmacists as needed.
• Prepare discharge paperwork, including instructions and prescriptions.
• Order diagnostic tests and referrals for further evaluation when necessary.
• Communicate effectively with patients, families, referring physicians, and clinical staff to ensure quality care.
• Participate in quality improvement, safety initiatives, and utilization reviews to enhance patient outcomes and cost-effectiveness.
• Alert physicians promptly to critical changes in patient status or abnormal test results.
• Maintain compliance with medical staff policies and accurate health information management.

Education and Experience:

• Completion or near completion (within 4 months) of a graduate program accredited by a recognized accrediting body, or completion of a certificate program with national certification.
• Board certification must be obtained and maintained within two years of program completion.

Licensure:

• Current, unrestricted Registered Nurse license to practice in the relevant state.
