Description

• Conduct thorough evaluations in inpatient and outpatient settings and develop individualized, goal-oriented treatment plans
• Deliver PT interventions targeting mobility, strength, functional movement, and balance restoration
• Work side by side with interdisciplinary care teams including physicians, nursing staff, case managers, and fellow rehabilitation specialists
• Guide patients and their support networks through therapeutic goals, home programs, and discharge planning strategies
• Maintain accurate, timely documentation of evaluations, session notes, and patient progress in compliance with facility and regulatory standards
• Engage in team rounds and interdisciplinary care meetings to ensure seamless, coordinated care delivery

Education

• Doctorate in Physical Therapy (DPT) or equivalent graduate degree from an accredited institution

Certifications

• Active state licensure as a Physical Therapist in Minnesota
• CPR certification preferred
• Additional specializations such as neurological or geriatric certifications are welcomed but not mandatory
