Job Summary: We are seeking a skilled Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join a full-time outpatient primary care team serving a diverse patient population in the Midwest region. This role supports a busy, collaborative Internal Medicine and Pediatrics practice, focusing mainly on acute and sick visits while also delivering comprehensive preventive and follow-up care across all ages.

Key Responsibilities:

• Evaluate and manage acute and sick visits for both pediatric and adult patients.
• Perform well child checks and sports physicals.
• Conduct Medicare Annual Wellness Exams.
• Complete hospital follow-up visits for adults with complex medical conditions.
• Order and interpret diagnostic tests, initiate treatment plans, and provide patient education.
• Follow up on diagnostic test results, including those received after hours.
• Collaborate closely with physicians and support staff to ensure high-quality, team-based care.
• Document care accurately and efficiently in the electronic medical record system.
• Consult with on-site physicians as needed; physician support is always available.

Practice Environment:

• Outpatient-only setting with no inpatient responsibilities.
• No after-hours call requirements.
• Team-based model supporting multiple Internal Medicine and Pediatric physicians.
• Collaborative team includes experienced Advanced Practice Providers and robust clinical and administrative support.
• On-site physician supervision and consultation available at all times.

Education and Experience:

• Graduate of an accredited Physician Assistant or Nurse Practitioner program.
• Board certified and eligible for licensure in the region.
• Preferred minimum of 2 years’ experience with a strong background in both adult and pediatric care (Family Medicine).
• Proven ability to manage complex patients and thrive in a high-volume clinical environment.
• Excellent communication, teamwork, and clinical decision-making skills.
• Commitment to maintaining required continuing medical education.

Licensure and Certifications:

• Certification from the National Commission of Physician Assistants or equivalent for Nurse Practitioners.
• Licensed to practice in the applicable state.

Work Schedule: Full-time, first shift, 40 hours per week.
