General Summary
Under the guidance of the Director of Care Management, the Associate Community Health Worker (CHW) supports patients in accessing and navigating community services. This role involves outreach, providing information and support to community members, and assisting with data collection to identify health needs and promote improvements, especially focusing on high-risk populations.

Essential Duties and Responsibilities
Build and maintain relationships with community partners, patients, and families while promoting healthy behaviors. Assist families, groups, and communities in accessing social and health-related services. Evaluate client needs and refer them to appropriate community resources and agencies, developing processes to assess Social Determinants of Health across various settings. Participate in community meetings and events such as health fairs to understand local issues and share resource information. Advocate for individual and community health needs. Serve as a liaison between healthcare providers, community organizations, and the populations served. Develop referral and follow-up processes for patient needs. Distribute educational materials tailored to the target population. Perform other duties as needed to support organizational goals.

Education
Associate degree in any field is required.

Experience
Experience with social services or public health systems is preferred.

Certification/License
Basic Life Support (BLS) certification must be obtained within the first 90 days of employment (training provided). Certification as a Community Health Worker must be earned within the first six months.

Mental/Physical Requirements
This position requires considerable mental focus for extended periods with frequent interruptions. Light lifting up to 15 lbs is necessary. Activities include standing, sitting, walking, reading, and speaking.

Working Conditions
Typical office environment with some noise and distractions. Possible eye strain from computer use. Attendance at public meetings is required. There may be exposure to infectious or communicable diseases when interacting with patients in various settings.
