Key Responsibilities

• Participate in agency councils, committees, or serve as a clinical coach, supporting professional development and quality care.
• Administer medications and perform planned interventions safely and efficiently, following established policies and protocols.
• Communicate patient condition changes effectively to physicians, advocating for patient needs using structured communication methods.
• Collaborate with healthcare team members and facilities to coordinate comprehensive patient care.
• Adapt positively to the dynamic healthcare environment and document patient care activities accurately using electronic medical records.
• Evaluate patient progress toward goals and update care plans accordingly, involving patients and families in the process.
• Provide patient and family education tailored to individual needs, educational levels, and cultural backgrounds.

Education

Graduation from an accredited nursing program is required. A Bachelor of Science in Nursing is preferred.

Experience

At least one year of clinical nursing experience is required, with preference given to candidates with home health nursing experience.

Certifications and Licenses

Current RN license in the state or compact licensure is required. New graduates must obtain licensure within 90 days of graduation. Basic Life Support certification from the American Heart Association is mandatory.

Physical and Mental Requirements

This role demands considerable mental focus due to the sensitive nature of end-of-life care. Physical activities include lifting up to 35 pounds, frequent walking, standing, turning, carrying, pushing, pulling, stooping, crouching, twisting, and reaching.

Working Conditions
