General Summary: The Acuity Care Management Coordinator serves as the primary triage contact within the care management department. This role involves directing patient discharge planning needs to the appropriate team members, supporting assigned tasks, acting as a liaison between care coordination teams, and assisting with clinical duties related to care management and denial prevention.

Essential Duties and Responsibilities:

• Serve as the main point of contact for nursing staff and care managers regarding discharge needs, including patient visits to assess discharge requirements.
• Assist with referral communications for pre- and post-hospital care and coordinate discharge-planning information with third-party payors and other healthcare providers.
• Support requests for Home Health Durable Medical Equipment (DME) by obtaining necessary order copies.
• Manage triage for acute and non-acute care areas, ensuring follow-up for discharge planning and transportation needs, including cost analysis of these services.
• Handle hospitalist calls requiring clarification from pharmacies or other providers.
• Track care manager assignments and assist denial prevention staff with administrative tasks such as faxing and printing.
• Verify benefits for post-acute care and hospital transfers.
• Work efficiently under pressure with strong attention to detail and clear communication across diverse individuals.
• Utilize community resources to support patient needs effectively.

Education: High School Diploma required.

Experience: A minimum of two years clinical experience in nursing, utilization management, social services, or quality management is preferred.

Certification/License: Current Basic Life Support (BLS) certification through the American Heart Association is required.

Mental/Physical Requirements: This position demands considerable mental focus. Physical activities include lifting up to 35 pounds, frequent walking, standing, turning, carrying, pushing, pulling, stooping, crouching, twisting, and reaching.

Working Conditions: Frequent exposure to infectious diseases and occasional encounters with hostile individuals. Exposure to communicable diseases through blood and bodily fluids is common. Minimal physical discomfort is expected.
