This role involves conducting patient assessments, delivering therapeutic interventions, and collaborating closely with interdisciplinary teams to support patient recovery and wellness.

Key Responsibilities

• Perform accurate patient assessments and implement treatment plans as directed by the Physical Therapist.
• Coordinate regularly with the supervising Physical Therapist to adjust care plans based on patient progress or changes in condition.
• Participate actively in interdisciplinary care planning to ensure comprehensive patient support.
• Communicate promptly any changes in patient status to the appropriate healthcare team members.
• Deliver cost-effective physical therapy services, managing resources efficiently.
• Educate patients and caregivers on therapy plans and evaluate the effectiveness of education provided.
• Complete all required documentation accurately and within designated timeframes.
• Manage daily tasks and time effectively to meet productivity goals.

Education

Graduation from an accredited Physical Therapist Assistant program is required.

Experience

• At least one year of experience in a Joint Commission accredited hospital setting.
• Minimum one year experience in an acute rehabilitation facility or equivalent, focusing on adult patient care.
• Home care experience of six months is preferred.
• Computer proficiency is a plus.

Certifications and Licenses

• Current Physical Therapist Assistant license valid in the state of practice.
• American Heart Association Basic Life Support (BLS) certification.
• Valid driver’s license and automobile liability insurance.

Physical and Mental Requirements

• Ability to stand and walk for extended periods and navigate stairs.
• Strong communication skills, both oral and written, with good visual acuity.
• Excellent interpersonal and organizational abilities.
• Capability to lift, transfer, and assist patients weighing between 10 and over 200 pounds.
• Safe operation of a motor vehicle.
