Join a dynamic healthcare team as an Acuity Care Management Coordinator, serving as the vital triage point within the care management department. In this role, you will coordinate patient discharge planning by directing needs to the appropriate team members, supporting clinical tasks, and acting as a liaison between care coordination teams. Your responsibilities include visiting patients to assess discharge requirements, managing referral communications for pre- and post-hospital care, and facilitating information exchange with third-party payors and care providers. You will also assist with home health equipment requests, triage non-acute care areas for discharge planning follow-up, and coordinate transportation services while analyzing associated costs. Additional duties involve clarifying hospitalist communications with pharmacies and providers, tracking care manager assignments, and supporting denial prevention staff with administrative tasks.

Education: A high school diploma is required.

Experience: Preferably, candidates will have at least two years of clinical experience in nursing, utilization management, social services, or quality management.

Certification/License: Current Basic Life Support (BLS) certification through the American Heart Association is mandatory.

Mental/Physical Requirements: This position demands considerable mental focus and physical activity, including lifting up to 35 pounds, frequent walking, standing, turning, carrying, pushing, pulling, stooping, crouching, twisting, and reaching.
