Join a dedicated healthcare team in the heart of the Midwest region, where your role as a Community Health Worker (CHW) will be vital in connecting patients with essential community services. Reporting to the Director of Care Management, you will engage with diverse populations, focusing on high-risk groups, to provide outreach, support, and advocacy.

Key Responsibilities

• Build and maintain strong relationships with community partners, patients, and families to promote healthy behaviors.
• Assist individuals, families, and groups in accessing social and health-related resources.
• Evaluate client needs and refer them to appropriate community agencies, incorporating Social Determinants of Health assessments.
• Participate in community events such as health fairs to stay informed on local issues and share resource information.
• Advocate for both individual and community health needs, acting as a liaison between healthcare providers and community organizations.
• Develop and manage referral and follow-up processes to ensure patient needs are met.
• Distribute educational materials tailored to the target populations served.
• Perform additional duties as needed to support organizational goals.

Education

An associate degree in public health, social work, psychology, sociology, or related fields such as communications, education, or business is required.

Experience

Previous experience in social services or public health systems is preferred.

Certifications and Licenses

• Basic Life Support (BLS) certification must be obtained within the first 90 days of employment; training is provided.
• Certification as a Community Health Worker must be earned within the first six months of employment.

Physical and Mental Requirements

This role requires sustained mental focus with frequent interruptions, light lifting up to 15 lbs, and the ability to stand, sit, walk, read, and communicate effectively. Typical office conditions include noise and distractions, with potential eye strain from computer use. Attendance at public meetings and possible exposure to infectious diseases during patient interactions are also part of the job.
