Description

• Conduct thorough evaluations for patients in both inpatient and outpatient settings, creating personalized treatment plans
• Implement physical therapy interventions with a focus on mobility, strength, balance, and overall function
• Partner with a diverse team of nurses, physicians, case managers, and other rehabilitation experts to optimize patient outcomes
• Educate patients and their families regarding therapeutic goals, discharge planning, and home exercise strategies
• Maintain detailed documentation of evaluations, treatments, and patient progress complying with facility and regulatory standards
• Engage in multidisciplinary conferences and care team rounds to ensure coordinated patient care

Education

• Doctorate in Physical Therapy (DPT) or a relevant equivalent from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification is preferred
• Additional certifications in specialized areas (neurological, geriatric) are advantageous but not mandatory
