Join a dynamic healthcare organization in the Midwest region as a ToRCH Program Manager, where you will play a pivotal role in developing and implementing impactful community health initiatives. Reporting to the Executive Director of Organizational Development and Learning, you will lead the ToRCH team to ensure program goals are achieved while fostering strong partnerships with local community resources.
Key Responsibilities
- Provide daily leadership and management for the ToRCH department, guiding staff to meet program objectives.
- Coordinate meetings, training sessions, and support for both internal and external stakeholders involved in the ToRCH program.
- Develop and execute community-wide strategies to address Social Determinants of Health (SDOH) for patients screening positive.
- Build and maintain relationships with community partners, Medicaid managed care plans, and organizations such as Unite Us and Aging Best to enhance patient resources.
- Participate in subcommittees focused on evaluating program success and goal achievement.
- Plan and manage community events related to the ToRCH program.
- Address internal customer relations challenges and proactively identify areas for service improvement.
- Perform additional duties as assigned.
Education
A Bachelor’s or Master’s degree in public health, social work, psychology, sociology, communications, or a related field is preferred. Candidates without a Bachelor’s degree must hold an Associate’s degree or have completed at least 60 college credit hours and commit to pursuing a Bachelor’s degree within one year of hire.
Experience
Minimum of two years’ experience in social work or care management is required. Experience in community health outreach or rural health improvement initiatives is highly desirable. Proven management experience in a related field is essential. Strong communication skills, project and program management abilities, technical report preparation, and advanced computer proficiency are necessary.
Mental and Physical Requirements
This role requires considerable mental focus for extended periods with frequent interruptions. Light lifting up to 15 lbs., standing, sitting, and walking are part of the daily routine.
