As an Acute Rehab Patient Care Assistant, you will play a vital role in supporting patient safety and comfort in a dynamic clinical environment.

Key Responsibilities

• Adhere to established safety protocols including infection prevention, safe patient handling, and timely communication of patient condition changes to licensed nurses.
• Perform basic patient care tasks such as monitoring vital signs, glucometer checks, assisting with toileting, feeding, bathing, and mobility activities.
• Collaborate closely with nursing staff to meet patient needs promptly, ensuring a safe and positive patient experience.
• Accurately document all patient care activities to support clinical decision-making and quality outcomes.
• Effectively communicate relevant patient information to healthcare team members to enhance care coordination.

Education

High school diploma or GED is preferred.

Experience

At least one year of experience in a patient care role is preferred.

Requirements

• Certified Nursing Assistant (CNA) certification through the state is preferred; if uncertified or with less than one year of experience, completion of a PCA training academy is required.
• American Heart Association Basic Life Support (BLS) certification must be obtained within the first 90 days of employment; training is provided.
• Physical demands include lifting up to 35 lbs., frequent walking, standing, bending, and other mobility tasks.
• Work environment involves exposure to infectious and communicable diseases and occasional interaction with challenging individuals.
