Join a dedicated healthcare team in the heart of the Midwest region, where your role as a Community Health Worker (CHW) will be vital in connecting patients with essential community services. Reporting to the Director of Care Management, you will engage in outreach efforts, provide support and information to community members, and assist in data collection to identify health needs and improve processes, especially focusing on high-risk populations.

Key Responsibilities

• Build and maintain strong relationships with community partners, patients, and families while promoting healthy behaviors.
• Assist individuals, families, and groups in accessing social and health-related services.
• Assess client needs and refer them to appropriate community resources and agencies, developing processes to evaluate Social Determinants of Health across various settings.
• Participate in community meetings and events such as health fairs to stay informed about local issues and share resource information.
• Advocate for the health needs of individuals and the broader community.
• Serve as a liaison between healthcare providers, community organizations, and the populations served.
• Develop and manage referral and follow-up processes for patient needs.
• Distribute educational materials tailored to the target populations.
• Perform additional duties as needed to support organizational goals.

Education

Associate degree in any field is required.

Experience

Preferred experience includes working with social services or public health systems.

Certifications and Licenses

• Basic Life Support (BLS) certification must be obtained within the first 90 days of employment; training is provided.
• Certification as a Community Health Worker must be earned within the first six months of employment.

Mental and Physical Requirements

This role requires considerable mental focus for extended periods with frequent interruptions. Light lifting up to 15 lbs is necessary. The position involves standing, sitting, walking, reading, and speaking regularly.

Working Conditions
