Join a dedicated healthcare team as a Homecare Occupational Therapist (OT) providing compassionate and personalized care to patients in a regional setting within the Midwest. This role involves developing individualized care plans after thorough patient evaluations, aiming to meet both patient and physician goals. You will work with home health and hospice patients, collaborating closely with interdisciplinary teams to ensure comprehensive care.

Essential Duties and Responsibilities

• Evaluate patients for home health admission based on CMS or payer guidelines and agency policies.
• Conduct initial and comprehensive assessments within required timeframes, documenting findings accurately.
• Create age-appropriate, individualized care plans focusing on self-management with measurable goals and interventions.
• Update care plans promptly to address new or resolved issues.
• Communicate progress and any emerging concerns with physicians and care team members.
• Include patients and caregivers in care and discharge planning, tailoring education to their learning preferences and barriers.
• Manage care delivery efficiently, including scheduling visits and utilizing supplies cost-effectively.
• Complete all documentation and revisions within designated timeframes.
• Supervise Certified Occupational Therapy Assistants (COTA) and homecare assistants as applicable, ensuring alignment with care plans.
• Collaborate with multidisciplinary teams through meetings and direct communication to optimize patient outcomes.
• Assist patients in obtaining necessary equipment and begin discharge planning upon admission.
• Provide treatments following physician orders and agency protocols, adhering to infection control and hand hygiene standards.

Education

Graduation from an accredited Occupational Therapy program is required.

Experience

• At least one year of experience in a JCAHO-accredited hospital.
• Minimum one year in an acute rehabilitation facility or equivalent, serving adult populations.
• Home care experience of six months is preferred.
• Computer skills are advantageous.

Certification and Licenses

• Current certification from the American Occupational Therapy Association (AOTA).
• Valid Occupational Therapist license in the relevant state.
• American Heart Association CPR certification.
• Current driver’s license and automobile liability insurance.

Mental and Physical Requirements

• Ability to stand and walk for extended periods and navigate stairs.
• Strong communication skills, both oral and written, with good visual acuity.
• Excellent interpersonal and organizational abilities.
• Capability to safely operate a vehicle for 2-4 hours daily.
• Ability to maneuver around home architectural barriers and tolerate environmental factors such as temperature changes, smoke, fumes, dust, and pets.
