Description

• Conduct thorough patient evaluations across inpatient or outpatient settings and develop individualized, goal-driven treatment plans
• Deliver physical therapy interventions targeting mobility restoration, muscular strength, balance training, and functional independence
• Work alongside physicians, nursing staff, case managers, and fellow rehabilitation specialists to drive optimal patient outcomes
• Guide patients and their families through therapeutic goals, home exercise programming, and discharge readiness
• Maintain thorough, timely documentation of evaluations, session notes, and patient progress in compliance with facility and regulatory standards
• Engage actively in interdisciplinary rounds and care conferences to ensure seamless, coordinated treatment across all disciplines

Education

• Doctor of Physical Therapy (DPT) or an equivalent graduate degree from a regionally accredited institution

Certifications

• Active state licensure as a Physical Therapist in good standing
• Current CPR certification preferred
• Specialty certifications in areas such as geriatrics, neurology, or orthopedics are welcomed but not required

Skills

• Prior clinical experience in a rehabilitation or acute care environment is strongly preferred
• Demonstrated ability to manage patients presenting with complex medical histories and significant mobility challenges
