Role Responsibilities

• Perform thorough physical examinations, order and interpret diagnostic studies, and craft individualized care plans.
• Work in close partnership with care coordinators, behavioral health specialists, and consulting physicians to ensure seamless, integrated care.
• Actively contribute to quality improvement and patient safety initiatives that drive measurable clinical outcomes.
• Maintain thorough, timely documentation through electronic medical records (EMR) systems.

Education Requirements

• MD or DO degree from an accredited medical school.
• Successful completion of an accredited Family Medicine residency program.

Certifications

• Board Certification in Family Medicine (or active board eligibility).
