Description

• Partner with a collaborative care team that includes physicians, nursing staff, case managers, and fellow rehabilitation specialists to drive optimal patient outcomes
• Guide patients and their families through therapeutic goals, home exercise programming, and safe discharge transitions
• Maintain accurate, thorough clinical documentation in alignment with facility standards and regulatory requirements
• Contribute to interdisciplinary rounds and care conferences to ensure seamless, coordinated patient care

Education

• Doctorate in Physical Therapy (DPT) or equivalent accredited degree

Certifications

• Active state licensure as a Physical Therapist in North Carolina
• CPR certification preferred
