Description

• Conduct thorough patient evaluations across inpatient or outpatient settings and develop personalized, goal-driven treatment plans
• Deliver targeted physical therapy interventions designed to improve mobility, functional strength, balance, and independence
• Partner with a multidisciplinary team including physicians, nursing staff, case managers, and fellow rehab specialists to deliver cohesive, high-quality care
• Provide meaningful education to patients and their families around therapeutic goals, home exercise programs, and safe discharge transitions
• Maintain thorough and timely documentation of evaluations, progress notes, and treatment records in compliance with regulatory and facility standards
• Contribute actively to interdisciplinary care rounds and team conferences to ensure continuity and coordination of patient care

Education

• Doctorate in Physical Therapy (DPT) or equivalent accredited degree required

Certifications

• Active state licensure as a Physical Therapist in New Mexico
• CPR certification preferred
• Specialty certifications in areas such as geriatrics, orthopedics, or neurology are welcomed but not required

Skills

• Prior clinical experience strongly preferred
• Demonstrated ability to manage patients presenting with complex medical and mobility challenges
• Exceptional interpersonal and communication skills with a collaborative mindset
• Proficiency with electronic health records and digital clinical documentation platforms
