Description

• Conduct thorough evaluations of patients in both inpatient or outpatient settings and tailor personalized treatment plans.
• Implement evidence-based physical therapy interventions aimed at improving mobility, strength, balance, and overall function.
• Work cooperatively with nurses, physicians, case managers, and other rehabilitation experts to enhance patient outcomes.
• Educate patients and their families about therapeutic goals, discharge planning, and necessary home exercises.
• Maintain accurate documentation of evaluations, treatments, and patient progress in line with facility and regulatory standards.
• Engage in care team discussions and interdisciplinary conferences to ensure a comprehensive approach to patient care.

Education

• Doctorate in Physical Therapy (DPT) or a similar degree from an accredited institution.

Certifications

• Valid state licensure as a Physical Therapist
• CPR certification is preferred
• Additional specialized certifications (e.g., neurological, geriatric) are beneficial but not mandatory.
