Description

• Conduct thorough patient evaluations across inpatient or outpatient settings and develop individualized, goal-oriented treatment plans
• Deliver targeted physical therapy interventions addressing strength, mobility, balance, coordination, and functional independence
• Partner with a multidisciplinary team — including physicians, nurses, case managers, and fellow rehabilitation specialists — to drive optimal patient outcomes
• Guide patients and their support systems through therapeutic goals, home exercise programming, and discharge preparation
• Maintain thorough, timely documentation of evaluations, session notes, and progress reports in alignment with facility and regulatory standards
• Actively contribute to interdisciplinary care conferences and team rounds to ensure seamless, coordinated care delivery

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• Valid state licensure as a Physical Therapist
• Current CPR certification preferred
• Advanced or specialty certifications — such as neurological or geriatric rehab — are welcomed but not required

Skills

• Prior clinical experience in a related setting is strongly preferred
• Demonstrated ability to manage patients presenting with complex medical conditions and significant mobility challenges
