Description

• Conduct comprehensive patient evaluations in inpatient or outpatient settings and develop individualized, goal-oriented treatment plans
• Deliver physical therapy interventions targeting mobility restoration, muscular strength, postural balance, and functional independence
• Partner closely with interdisciplinary care teams including physicians, nursing staff, case managers, and fellow rehabilitation professionals to drive optimal patient outcomes
• Provide patient and family education covering therapeutic objectives, safe home exercise programs, and discharge readiness
• Maintain thorough and timely documentation of evaluations, session notes, and patient progress in alignment with facility protocols and regulatory standards
• Actively participate in multidisciplinary rounds and care conferences to ensure seamless, coordinated patient care

Education

• Doctor of Physical Therapy (DPT) or equivalent degree awarded by an accredited institution

Certifications

• Active state licensure as a Physical Therapist in New York
• CPR certification preferred
• Advanced or specialty certifications such as neurological rehab or geriatric physical therapy are welcomed but not required

Skills

• Prior clinical experience strongly preferred, though motivated newer graduates are encouraged to apply
