Role Responsibilities

• Deliver full-spectrum, continuous care to patients and families across all age groups, from pediatric through geriatric populations.
• Diagnose and manage chronic diseases, acute conditions, preventive health needs, and behavioral health concerns using a patient-first framework.
• Perform thorough physical examinations, order and interpret diagnostic studies, and craft individualized care plans tailored to each patient's unique circumstances.
• Work in close partnership with care coordinators, behavioral health specialists, and subspecialty consultants to ensure seamless, integrated care delivery.
• Actively contribute to quality improvement and clinical innovation initiatives designed to elevate both patient outcomes and team performance.
• Maintain precise, thorough documentation within the electronic medical record system to support continuity and compliance.
• Extend your reach through both in-person visits and virtual care to meet patients wherever they are.

Education Requirements

• MD or DO degree from an accredited medical school.
• Successful completion of an accredited Family Medicine residency program.

Certifications

• Board Certification in Family Medicine (or active board eligibility).
• Active New York medical license or eligibility to obtain one.
• BLS and ACLS certifications preferred.
