What You Will Be Doing

• Performing thorough patient assessments, ordering and interpreting diagnostic testing, and developing personalized, evidence-informed care plans
• Diagnosing and managing acute, episodic, and chronic conditions — independently or in collaboration with attending physicians based on credentialing and scope
• Serving as a trusted educator for patients and their families, guiding them through prevention strategies, wellness goals, and chronic disease management
• Prescribing medications and therapeutic interventions within your full scope of practice
• Leading and participating in follow-up care, care coordination, and longitudinal chronic condition management
• Collaborating fluidly with interdisciplinary care teams to ensure patients receive seamless, whole-person support
• Maintaining detailed, accurate, and timely clinical documentation within electronic medical record systems

Education Requirements

• Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited institution
• Completion of an accredited Nurse Practitioner program

Required Certifications

• Active Nurse Practitioner licensure in the state of New York
• National board certification as a Family Nurse Practitioner (FNP-C via AANP or ANCC) or specialty-equivalent credential
• Current BLS and ACLS certifications, or the ability to obtain both within 90 days of hire
