Description

• Perform thorough patient evaluations in inpatient or outpatient settings and develop individualized, goal-driven treatment plans
• Deliver targeted physical therapy interventions addressing mobility, muscular strength, coordination, and functional independence
• Work alongside physicians, nurses, case managers, and rehabilitation specialists to deliver seamless, patient-centered care
• Guide patients and their families through therapeutic goals, home exercise programming, and discharge preparation
• Maintain accurate and timely documentation of evaluations, sessions, and patient progress in compliance with facility and regulatory standards
• Actively engage in interdisciplinary rounds and care conferences to support coordinated recovery journeys

Education

• Doctorate in Physical Therapy (DPT) or equivalent accredited degree required

Certifications

• Active Physical Therapist licensure in the state of New York
• CPR certification preferred
• Specialty certifications in areas such as geriatrics, neurology, or orthopedics are welcomed but not mandatory

Skills

• Prior clinical experience strongly preferred
• Demonstrated ability to manage patients with complex, multifaceted medical and mobility challenges
