Deliver Patient Care:

• Perform comprehensive physical assessments and gather patient histories.
• Diagnose a wide range of acute and chronic health conditions.
• Order and interpret diagnostic tests, including labs and imaging.
• Develop and execute personalized treatment plans.
• Prescribe medications, manage chronic illnesses, and provide follow-up care.

Patient Education and Support:

• Educate patients and families about diagnoses, medications, and lifestyle changes.
• Provide preventative services including wellness screenings and immunizations.
• Offer guidance on long-term health management and self-care strategies.

Collaborate with Healthcare Teams:

• Work alongside physicians, specialists, and care teams to ensure coordinated treatment.
• Participate in clinical discussions, case reviews, and treatment planning.
• Support nursing staff and contribute to a positive, collaborative work culture.

Monitor Progress & Ensure Compliance:

• Evaluate treatment outcomes and adjust plans as needed.
• Maintain accurate documentation in EHR systems.
• Stay current on medical best practices and comply with all healthcare regulations and privacy laws.

Qualifications:

• Master’s degree in Nursing from an accredited Nurse Practitioner program.
• Active Nurse Practitioner (NP) license in Roslyn Heights State.
• Board Certification (e.g., FNP, AGNP, etc.) required.
• BLS and ACLS certifications preferred.

• Experience in primary care, urgent care, or specialty medicine preferred (new grads welcome).
