Description

• Perform thorough patient evaluations across inpatient or outpatient settings and develop individualized, goal-oriented treatment plans
• Deliver physical therapy interventions targeting functional mobility, muscular strength, balance restoration, and overall independence
• Engage in active collaboration with interdisciplinary teams — including physicians, nurses, case managers, and fellow rehab specialists — to drive optimal patient outcomes
• Provide meaningful education to patients and their families regarding therapeutic goals, home exercise programs, and discharge preparation
• Maintain accurate, timely documentation of evaluations, treatments, and patient progress in alignment with facility and regulatory standards
• Actively contribute to care conferences and team rounds to support seamless, coordinated care across settings

Education

• Doctorate in Physical Therapy (DPT) or equivalent accredited degree required

Certifications

• Active Physical Therapist licensure in the state of Ohio
• CPR certification preferred
