Description

• Conduct thorough patient evaluations across inpatient or outpatient settings and develop individualized, goal-directed treatment plans
• Deliver physical therapy interventions targeting functional mobility, muscular strength, postural balance, and overall independence
• Work alongside a multidisciplinary care team — including physicians, nursing staff, case managers, and fellow rehabilitation specialists — to drive optimal patient outcomes
• Guide patients and their families through therapeutic goals, safe home exercise programs, and discharge planning strategies
• Maintain accurate and timely clinical documentation aligned with facility protocols and regulatory standards
• Actively contribute to interdisciplinary rounds and care conferences to ensure continuity and cohesion across the patient journey

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• Active state licensure as a Physical Therapist in good standing
• CPR certification is preferred
• Additional specialty certifications — such as neurological or geriatric rehabilitation — are welcomed but not required

Skills

• Prior clinical experience is strongly preferred
• Demonstrated ability to manage patients presenting with complex medical conditions and mobility challenges
• Excellent interpersonal communication and collaborative team skills
• Proficiency with electronic health records and digital documentation platforms
