Description

• Conduct thorough patient evaluations across inpatient and outpatient settings and develop individualized, goal-oriented treatment plans
• Deliver evidence-based physical therapy interventions targeting mobility, strength, balance, coordination, and functional independence
• Work alongside physicians, nurses, case managers, and fellow rehabilitation professionals to ensure seamless, patient-centered care
• Guide patients and their families through therapeutic goals, home exercise programs, and discharge preparation
• Maintain thorough, timely documentation of assessments, treatments, and patient progress in alignment with facility and regulatory standards
• Actively engage in interdisciplinary rounds and care conferences to promote holistic, coordinated outcomes

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• Active state licensure as a Physical Therapist in good standing
• CPR certification is preferred
