Description

• Conduct thorough patient evaluations across inpatient and outpatient settings and develop individualized, goal-oriented treatment plans
• Deliver physical therapy interventions targeting strength, mobility, balance, coordination, and functional independence
• Partner closely with physicians, nurses, case managers, and fellow rehabilitation professionals to drive optimal patient outcomes
• Provide education to patients and their support systems regarding therapeutic goals, home exercise programs, and discharge planning strategies
• Maintain accurate, timely documentation of evaluations, session notes, and progress in compliance with facility standards and regulatory requirements
• Actively contribute to interdisciplinary care conferences and rounding to ensure seamless, coordinated care delivery

Education

• Doctorate in Physical Therapy (DPT) or equivalent degree from an accredited institution

Certifications

• Active Physical Therapist licensure in the state of Pennsylvania
• CPR certification preferred
• Specialty certifications in areas such as geriatrics, neurology, or orthopedics are welcomed but not required

Skills

• Prior clinical experience is strongly preferred but not a barrier for the right candidate
• Demonstrated ability to manage patients presenting with complex medical histories and mobility challenges
