Description

• Conduct thorough patient evaluations across inpatient or outpatient settings and build personalized, goal-driven treatment plans
• Deliver targeted physical therapy interventions addressing mobility, functional strength, balance, and coordination
• Work alongside a multidisciplinary team of physicians, nurses, case managers, and rehabilitation specialists to deliver seamless, coordinated care
• Provide education and guidance to patients and their families regarding therapeutic milestones, home exercise programming, and safe discharge transitions
• Maintain accurate, timely documentation of evaluations, sessions, and patient progress in alignment with facility and regulatory standards
• Actively participate in interdisciplinary care conferences and rounding to ensure every patient receives holistic, patient-centered treatment

Education

• Doctorate in Physical Therapy (DPT) or equivalent accredited degree required

Certifications

• Active Physical Therapist licensure in the state of practice
• CPR certification preferred
