Description

• Conduct thorough patient evaluations across inpatient and outpatient settings and develop individualized, goal-oriented treatment plans
• Deliver targeted physical therapy interventions designed to restore mobility, strength, balance, and overall functional independence
• Work closely with a multidisciplinary team that includes physicians, nurses, case managers, and fellow rehabilitation specialists to drive optimal patient outcomes
• Guide patients and their families through therapeutic objectives, safe home exercise programs, and discharge readiness planning
• Maintain accurate, timely documentation of evaluations, clinical interventions, and patient progress in compliance with facility and regulatory standards
• Engage in interdisciplinary rounds and collaborative care conferences to ensure seamless, coordinated treatment across the continuum of care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from a regionally accredited institution

Certifications

• Active state licensure as a Physical Therapist in good standing
• CPR certification preferred
