Description

• Perform comprehensive patient evaluations across inpatient or outpatient settings and develop individualized, goal-driven treatment plans
• Deliver hands-on physical therapy interventions targeting strength, mobility, balance, pain management, and functional restoration
• Work in close partnership with physicians, nursing staff, case managers, and interdisciplinary rehab colleagues to drive optimal patient outcomes
• Guide patients and their families through therapeutic goals, home exercise programming, and safe discharge transitions
• Maintain thorough, timely clinical documentation consistent with facility protocols and regulatory standards
• Actively participate in care team huddles and interdisciplinary conferences to ensure seamless, coordinated care delivery

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• Active state licensure as a Physical Therapist in good standing
• CPR certification preferred
• Specialized credentials in areas such as neurology, geriatrics, or orthopedics are welcomed but not required

Skills

• Prior clinical experience strongly preferred
• Proven ability to manage patients presenting with complex medical histories and mobility challenges
