Description

• Conduct thorough patient evaluations across inpatient or outpatient settings and develop individualized, goal-driven treatment plans
• Deliver targeted physical therapy interventions addressing mobility, functional strength, balance, and coordination
• Partner with physicians, nursing staff, case managers, and fellow rehabilitation specialists to drive optimal patient outcomes
• Guide patients and their families through therapeutic goals, home exercise programs, and safe discharge planning
• Maintain thorough, timely documentation of assessments, interventions, and patient progress in compliance with facility and regulatory standards
• Engage in interdisciplinary rounds and care conferences to ensure seamless, coordinated care delivery

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• Active state licensure as a Physical Therapist in good standing
• CPR certification preferred
• Specialty certifications in areas such as geriatrics, neurology, or orthopedics are welcomed but not required
